Long Term Conditions

"Nothing for me, without me."
Shared decision making is part of any effective healthcare consultation.

Baxter has a strong track-record of providing solutions to long-term conditions; be it providing life saving and sustaining products, or understanding the patient pathway that allows a patient to seamlessly travel between hospital and the home to get the right therapy, at the right time.

We have drug and service experience in home dialysis, haemophilia, immunoglobulin therapy and provide tailored drug solutions for individual patients throughout hospitals in the UK.

Nutrition

In a complex clinical area where meeting a patient's individual parenteral nutritional requirements is imperative in any situation, we believe that Baxter is the right partner for healthcare professionals. We offer simplicity and ease of use to the customer while still providing many of the essential features that are required to deliver a seamless parenteral nutrition service for patients by our product offering. We can also support clinicians trying to tailor their parenteral nutrition feeds to meet the clinical requirements of the patient as well as supporting the smooth transition for parenteral nutrition patients from hospital to home with our unique personal homecare service through Willow.

Rare Diseases

Haemophilia is a rare disease that whilst affecting a relatively small patient population, has specific needs and represents a significant cost to the NHS. Baxter is well known for providing home treatment through our Factor Direct delivery service. We also have expertise in immunology, where we again enable seamless care between the hospital and home.

Scenario | NUTRITION:
Meeting nutritional requirements

Seven years ago, 39-year-old Pete suffered an acute mesenteric artery infarct necessitating emergency resection of most of his small bowel. With inadequate gut available to absorb the nutrients to meet his needs, he became dependent on parenteral nutrition. The clinical team selected his feeds from the diverse range of Baxter triple chamber nutrition bags, ensuring the optimum protein and energy balance, volume and electrolyte profile. In line with the NICE recommendations, they ensured micronutrients were included every day. Nursing and homecare support have enabled Pete to restore a relatively normal lifestyle; he's returned to work and enjoys cycling, while being confident to administer his feed at home each night.

Scenario | RARE DISEASES:
Living with Primary Immunodeficiency

Jane is mum of Dan, a happy little boy.  However, he wasn't always so happy. In just one year he had suffered from eight ear infections and he had pneumonia twice. Luckily, his GP knew the warning signs and referred him to a specialist for some tests.  The specialist diagnosed Dan with Primary Immunodeficiency.  Now with the help of immunoglobulins he leads a happy and healthy life again.
